Glucophage XR 500mg contains metformin hydrochloride in an extended-release form. This medication is intended to help control blood glucose levels in those with type 2 diabetes by improving how the body reacts to insulin and lowering the liver’s glucose production. Its extended-release formulation allows for slow absorption, which can lead to better tolerance and fewer gastrointestinal problems. Each tablet has 500mg of metformin hydrochloride.

Suggested Use
Glucophage XR 500mg is:
- Used as monotherapy or in combination with other antidiabetic medications or insulin to control blood sugar levels in adult and pediatric patients (10 years and older).
- Off-label use in managing symptoms associated with PCOS, such as insulin resistance.
Adults:
Start with 500mg once daily with the evening meal. Adjust the dose by increments of 500mg every 1–2 weeks based on blood glucose levels and tolerability. Maximum dose: 2000mg per day.
Children (10 years and older): Start with 500mg once daily with the evening meal. Adjust as needed, with a maximum dose of 2000mg daily.
Take with food to reduce gastrointestinal side effects. Swallow the tablet whole; do not crush, chew, or break it.
Warning
- Lactic Acidosis: A rare but serious complication that may occur due to metformin accumulation, particularly in patients with kidney impairment or other conditions that predispose to hypoxia. Symptoms include rapid breathing, abdominal pain, and weakness. Immediate medical attention is required.
- Contraindicated in patients with severe renal impairment (eGFR <30 mL/min/1.73m²). Assess renal function before initiating treatment and periodically thereafter.
- Temporarily discontinue before imaging procedures requiring iodinated contrast and resume after kidney function re-evaluation.
- Hypoglycemia: Rare when used alone but may occur when combined with insulin or sulfonylureas.
- Common side effects include nausea, diarrhoea, and abdominal discomfort. Starting with a low dose and gradually titrating can minimize these effects.
- Long-term use may reduce vitamin B12 absorption. Monitor levels in prolonged treatment.
- Contraindications: Severe renal or hepatic impairment, Acute or chronic metabolic acidosis, including diabetic ketoacidosis and Hypersensitivity to metformin or any excipient in the formulation.
