Coempaken 5/500mg Tablets are a combination medication that helps manage blood sugar levels in adults with Type 2 Diabetes Mellitus. By simplifying treatment, Coempaken enhances patient adherence and improves health outcomes compared to using separate medications. Active Substances: Empagliflozin 5mg and Metformin Hydrochloride 500mg per tablet.

Suggested Use

Coempaken is primarily used for:

  1. Managing high blood sugar levels in adults with Type 2 Diabetes Mellitus.
  2. Reducing the risk of cardiovascular events in patients with T2DM who also have existing heart disease.
  3. Supporting weight loss and improving renal function in diabetic patients.

 

Dosage:

The recommended dosage of Coempaken varies based on individual glycemic control needs and prior Metformin use. Typically, it is prescribed as:

  • 1 tablet taken twice daily (BID) with meals to minimize gastrointestinal side effects.
  • Dosage adjustments may be necessary depending on the patient’s response and tolerance to the medication

Warning

  1. Lactic Acidosis: Rare but serious risk associated with metformin use, especially in cases of renal dysfunction or conditions predisposing to hypoxia.
  2. Ketoacidosis: Empagliflozin can increase the risk of diabetic ketoacidosis; monitor patients closely for symptoms.
  3. Renal Function: Regular monitoring of kidney function is essential. Do not use if eGFR <30 mL/min/1.73 m².
  4. Volume Depletion: Empagliflozin may cause dehydration; maintain adequate fluid intake.
  5. Hypoglycemia: Risk increases when used with insulin or sulfonylureas.
  6. Vitamin B12 Deficiency: Long-term use of metformin may cause B12 deficiency; monitor periodically.
  7. Pregnancy and Lactation: Not recommended during pregnancy or breastfeeding.
  8. Contraindications:
    • Severe renal impairment (eGFR <30 mL/min/1.73 m²)
    • Acute or chronic metabolic acidosis
    • Known hypersensitivity to any of the components.

Side Effects

  • Common: Nausea, diarrhoea, urinary tract infections, and increased urination.
  • Rare but Serious: Lactic acidosis, diabetic ketoacidosis, and severe allergic reactions.
