NSAIDs
Suggested Use
- Management of chronic hyperuricemia in conditions where urate deposition has already occurred, such as gouty arthritis and tophi (uric acid crystal deposits).
- Prevention of gout flares during treatment for hyperuricemia.
Dosage:
Adults:
- The recommended starting dose is 40mg or 80mg once daily, depending on uric acid levels.
- If uric acid levels do not reach the target of less than 6 mg/dL (360 µmol/L) after 2–4 weeks, the dose may be increased to 120mg daily.
Note:
- Can be taken with or without food.
- Adjustments are typically not needed for mild to moderate kidney or liver impairment, but a healthcare provider should be consulted.
Warning
- Allergic Reactions:
- Discontinue if signs of hypersensitivity, such as rash, fever, or respiratory symptoms, occur.
- Cardiovascular Risks:
- Caution in patients with a history of cardiovascular disease; monitor for chest pain, stroke, or heart attack.
- Severe Gout Flares:
- Initial treatment may cause gout flares as uric acid levels fluctuate. Co-administration of an anti-inflammatory agent (e.g., NSAIDs or colchicine) is recommended for the first 6 months.
- Liver Function:
- Periodic liver function tests are recommended during treatment.
- Contraindications:
- Known hypersensitivity to febuxostat or any of the excipients.
- Use during pregnancy and lactation only if clearly needed and prescribed by a doctor.
- Drug Interactions:
- Caution with drugs metabolized by xanthine oxidase (e.g., azathioprine or mercaptopurine) as febuxostat can increase toxicity.
Side Effects:
- Common: Nausea, rash, joint pain, liver enzyme elevations.
- Rare but serious: Severe hypersensitivity reactions, cardiovascular events, or renal failure.
